RESUMO

BACKGROUND: Functional assessment of the future liver remnant (FLR) after major hepatectomy is essential but often difficult in patients with biliary malignancy, owing to obstructive jaundice and portal vein embolization. This study evaluated whether a novel index using gadoxetate disodium-enhanced MRI (EOB-MRI) could predict posthepatectomy liver failure (PHLF) after major hepatectomy for biliary malignancy. METHODS: The remnant hepatocellular uptake index (rHUI) was calculated in patients undergoing EOB-MRI before major hepatectomy for biliary malignancy. Receiver operating characteristic (ROC) curve analyses were used to evaluate the accuracy of rHUI for predicting PHLF grade B or C, according to International Study Group of Liver Surgery criteria. Multivariable logistic regression analyses comprised stepwise selection of parameters, including rHUI and other conventional indices. RESULTS: This study included 67 patients. The rHUI accurately predicted PHLF (area under the curve (AUC) 0.896). A cut-off value for rHUI of less than 0.410 predicted all patients who developed grade B or C PHLF. In multivariable analysis, only rHUI was an independent risk factor for grade B or C PHLF (odds ratio 2.0 × 103, 95 per cent c.i. 19.6 to 3.8 × 107; P < 0.001). In patients who underwent preoperative portal vein embolization, rHUI accurately predicted PHLF (AUC 0.885), whereas other conventional indices, such as the plasma disappearance rate of indocyanine green of the FLR and FLR volume, did not. CONCLUSION: The rHUI is potentially a useful predictor of PHLF after major hepatectomy for biliary malignancy.

RESUMO

The capability for actual measurements-not just simulations-of the dynamical behavior of THz electromagnetic waves, including interactions with prevalent 3D objects, has become increasingly important not only for developments of various THz devices, but also for reliable evaluation of electromagnetic compatibility. We have obtained real-time visualizations of the spatial evolution of THz electromagnetic waves interacting with a single metal micro-helix. After the micro-helix is stimulated by a broadband pico-second pulse of THz electromagnetic waves, two types of anisotropic re-emissions can occur following overall inductive current oscillations in the micro-helix. They propagate in orthogonally crossed directions with different THz frequency spectra. This unique radiative feature can be very useful for the development of a smart antenna with broadband multiplexing/demultiplexing ability and directional adaptivity. In this way, we have demonstrated that our advanced measurement techniques can lead to the development of novel functional THz devices.

RESUMO

The measurement of water concentration and distribution in thin biotissues with terahertz (THz)-wave has been proposed. In this paper, a novel sample preparation approach was introduced to effectively preserve tissue freshness at room temperature. Excellent stability of this method was demonstrated by measuring the transmittance spectroscopy and imaging many times within a certain time. Moreover, the reliability of water volume concentration measurement with THz-wave was evaluated. Measurement results using THz-wave were in good agreement with volume concentration measurement results based on other quantitative methods. The results suggest that water concentration and distribution measurement in thin biotissues using THz-wave will be a potential modality for medical and biological diagnosis.

RESUMO

Collective Thomson scattering (CTS) system has been constructed at LHD making use of the high power electron cyclotron resonance heating (ECRH) system in Large Helical Device (LHD). The necessary features for CTS, high power probing beams and receiving beams, both with well defined Gaussian profile and with the fine controllability, are endowed in the ECRH system. The 32 channel radiometer with sharp notch filter at the front end is attached to the ECRH system transmission line as a CTS receiver. The validation of the CTS signal is performed by scanning the scattering volume. A new method to separate the CTS signal from background electron cyclotron emission is developed and applied to derive the bulk and high energy ion components for several combinations of neutral beam heated plasmas.

RESUMO

Record-breaking high power coherent radiation at a subterahertz frequency region from a gyrotron utilizing second harmonic resonance modes was attained with a simple cavity. In order to aim at high power and high frequency simultaneously, the oscillation mode was selected carefully enough to realize stable radiation free from mode competition. The cavity radius was determined from the viewpoints of the oscillation frequency, the coupling coefficient between the electron beam, and the rf-electric field. The cavity length was also optimized for the highest perpendicular efficiency. In addition, a new electron gun which is capable of generating a thin laminar beam for a large current was introduced. Consequently, single mode second harmonic radiation with powers of 52 and 37 kW at frequencies of about 349 and 390 GHz, respectively, was achieved.

Collective Thomson scattering (CTS) system is expected to be a strong diagnostic tool for measuring thermal and fast ion distribution function at a local point inside plasmas. The electron cyclotron resonance heating system using a gyrotron at the frequency range of 77 GHz has been installed at the large helical device (LHD). The feasibility of CTS system using the 77 GHz gyrotron is assessed in terms of scattering spectrum and a background noise of the electron cyclotron emission, which affect the signal to noise ratio, with the realistic plasma parameters and incident port locations of LHD. Based on the calculated scattering spectra for bulk and tail fast ion diagnostics, the scattering radiation receiver system with gyrotron frequency feedback circuit is proposed to avoid the frequency chirping.

Collective Thomson scattering (CTS) is expected to provide the spatially resolved velocity distribution functions of not only thermal and tail ions but also alpha particles resulting from fusion reactions. CTS using gyrotrons with frequency higher than the conventional ones used for plasma heating would have advantages to alleviate refraction, cutoff effects, and background electron cyclotron emission noise. Therefore, a high-power pulse gyrotron operating at approximately 400 GHz is being developed for CTS in Large Helical Device (LHD). A single-mode oscillation with a frequency greater than 400 GHz, applying the second-harmonic resonance, was successfully demonstrated in the first stage. At the same time, concrete feasibility study based on ray tracing, scattering spectra, and electron cyclotron emission calculations has been conducted.

RESUMO

In the Large Helical Device plasma discharges, the size of an externally imposed island with mode number ( n/m = 1/1) decreases substantially when the plasma is collisionless ( nu(*)< approximately 1) and the beta is finite ( > approximately 0.1%) at the island location. For the collisional plasmas with finite beta, on the other hand, the size of the island increases. However, there is a threshold in terms of the vacuum island size below which the island enlargement is not seen.

RESUMO

It was observed that the vacuum magnetic island produced by an external error magnetic field in the large helical device shrank in the presence of plasma. This was evidenced by the disappearance of flat regions in the electron temperature profile obtained by Thomson scattering. This island behavior depended on the magnetic configuration in which the plasmas were produced.

RESUMO

Recent large helical device experiments revealed that the transition from ion root to electron root occurred for the first time in neutral-beam-heated discharges, where no nonthermal electrons exist. The measured values of the radial electric field were found to be in qualitative agreement with those estimated by neoclassical theory. A clear reduction of ion thermal diffusivity was observed after the mode transition from ion root to electron root as predicted by neoclassical theory when the neoclassical ion loss is more dominant than the anomalous ion loss.
